Case Management & Aftercare

About Case Management

All youth engaged with Gateway Youth Housing Services receive developmentally appropriate, individualized case management support designed to meet their unique needs and goals. Services are strengths-based, youth-driven, and
provided with sensitivity to each person’s background and experiences.

Housing Case Managers partner with youth experiencing housing instability to assess needs and develop goals around four key areas:

  • Housing Stability: Supporting youth in obtaining and maintaining safe, stable housing.
  • Health and Well-Being: Encouraging access to healthcare and resources that promote physical and emotional wellness.
  • Life Skills: Building everyday skills that promote greater independence, including financial literacy, budgeting, understanding lease agreements, preventing evictions, and connecting to local resources.
  • Supportive Relationships: Helping youth strengthen natural support systems and build positive connections with peers, mentors, and caring adults in the community.


Case Management addresses multiple areas of life that can affect housing stability, including:

  • Housing searches
  • Roommate communication
  • Applying for public benefits or assistance
  • Strengthening parenting skills
  • Referrals to other community services as needed

Aftercare

After a young person has stabilized in permanent housing and no longer requires financial assistance, Case Managers provide ongoing check-ins and support. These follow-ups are designed to help youth maintain housing, continue building their support networks, and further develop life skills to avoid future housing crises.
