Our Street Outreach and Crisis Prevention team focuses on proactive, street-based intervention to connect vulnerable youth and young adults with essential services before crises escalate. We engage with youth in public spaces, providing education about available programs and resources to support their stability and well-being.
Key Services Include
Early Assessment & Prevention: Identifying short- and long-term housing and service needs to prevent future instability.
Crisis Intervention: Trained staff connect youth to critical support services, including emergency shelters, crisis hotlines, and specialized resources for at-risk populations.
Basic Needs Support: When available, the team provides or facilitates access to food, hygiene items, weather-appropriate clothing, and infant care supplies.
Housing Navigation & Coordinated Entry: Helping youth enroll in the Coordinated Entry system to be prioritized for housing services.
Collaborative Housing Support: Partnering with local housing agencies to ensure seamless transitions into safe and stable housing.
Transportation Assistance: Providing transportation to safe shelter as needed. By reaching youth where they are, we focus on early intervention and stabilization, reducing the risk of long-term homelessness and crisis situations.
